Historically, healthcare provision has been closely linked to hospital infrastructures. In order to reconcile the current challenges of increasing demand for care, changing population catchment areas, and constraints in terms of human and financial resources, the supply of care is now considered on a regional scale, with a focus on care or health pathways. This paradigm shift calls for a more dynamic approach to tomorrow's healthcare, in which time management is a major objective, both for organizing flows and for sharing synchronized information in real time between healthcare professionals. Today, digital healthcare has all the assets needed to meet the challenge of moving from hospital stays to care paths. By presenting a case study on the organization of home hospitalization, we examine the role of technological innovation in organizational transformation projects.
